Q: What does Patina do, and what is your mission?
A: Patina is a technology-enabled healthcare solution that offers in-home and virtual care for adults 65 and older. Every patient has their own multidisciplinary team of primary care practitioners, nurses, health champions, and behavioral health specialists who deliver highly personalized support for their total medical, emotional, social and logistical care needs from the comfort and convenience of their home (or wherever they are). We believe that everyone should have the opportunity to live their best lives and age with dignity, with access to quality care that is human-centered and personalized.
Q: What process were you looking to improve with Metriport?
A: One of the ways we create a profoundly better experience for the people we serve is collecting, organizing, and reviewing all of their medical records before their first visit with their care team. This allows us to have a full picture of their healthcare situation prior to meeting them and use our time with them getting to know them, filling in gaps, and developing a personalized care plan.
Q: How did Metriport come into play?
A: With a need to improve this process in mind, we chose Metriport, an open-source solution for patient clinical histories. Through Metriport’s dashboard, we were able to do some initial data pulls to validate patients’ coverage across a large network of health information exchanges and EHR systems. This approach allowed us to see the value in Metriport’s solution before making any long term commitments to integrate their API into our workflows.

Q: How does Metriport fit into Patina's workflows?
A: After leveraging Metriport's dashboard, we integrated their API, which resulted in us getting patients’ medical histories before their first visit. We received these in minutes, as opposed to days under the prior manual method.
A big part of the value from Metriport came from their medical record summaries, which we generated through a single API call on top of their structured FHIR data. Since Metriport aggregates and de-duplicates thousands of patient records across all major HIEs and EHRs, these summaries allowed us to get a quick overview of our patients’ health histories in a format that was easily accessible to our care team.
Having access to structured data from Metriport enables us to automatically update patient records directly in our EMR and store the clinical data for analytics, suspecting, medication reconciliation and predictive modeling.
Q: How do you see this evolving in the future?
A: Using structured patient data from Metriport coupled with our AI modeling opens up many possibilities for us, and we look forward to seeing where this collaboration takes us in the future.
